By checking this box as the parent/legal guardian of the above-named participant, I understand and acknowledge that participation in the Wonder World Summer Camp involves physical activities, sports, and recreational events that may involve risks of injury. *

By checking this box, I hereby release, waive, and discharge Coach for Kids, Wonder World Summer Camp, Colegio Marista, its directors, staff, volunteers, and any associated entities from any and all liability, claims, demands, and causes of action arising out of or related to any loss, damage, or injury, that may be sustained by my child while participating in the camp or while on camp premises.*

By checking this box, I assume full responsibility for any risk of bodily injury, illness, or loss incurred due to my child’s participation in camp activities. I also acknowledge that I am responsible for informing the camp of any medical conditions, allergies, or special needs my child has before attending. I further authorize Wonder World Summer Camp staff to obtain medical treatment for my child in case of emergency if I cannot be reached. I agree to bear any costs related to such treatment.*
